The Booking Form is in pdf format and therefore cannot be completed online. Please print a copy of the form, complete all sections clearly and legibly, especially those concerning shore side contacts, doctor's contact details and the sections on any medical conditions and special dietary requirements. If under the age of 18, the form requires the signature of a parent, guardian or carer. Send the form by post to us at the address detailed at the top of the form, if it is urgent, it can be faxed to us by first telephoning on 07901 914499. NOTE TO SEA STAFF: If the personal details required on the form have not changed since you last sailed with us, there is no requirement to complete another form. However, if you are sailing with us as a volunteer member of sea staff for the first time or if you are on a familiarisation voyage, a form must be completed and sent to us one week prior to embarkation.
